Summary of Benefits and Coverage| A new requirement of the Affordable Care Act which allows you to view plan information for each medical option in a standard format
Qualifying Events Quick Reference Guide| Your ability to make benefit changes outside of Annual Enrollment is limited. You may make changes to your benefits only if you experience a Qualifying Event, which include: your marriage or divorce; birth or legal adoption of a child; death of a spouse or child; a change in your spouse's employment affecting your benefit coverage; or a change in child eligibility. If you need to make a change to your benefits mid- year due to one of these reasons, you must submit your online Qualifying Event request in HRconnect within 30 days from the date of the event. If you do not submit your request within 30 days, your request will be denied.
HSA Ineligibility Affidavit and FSA Enrollment Form| This form must be completed if you have indicated that you are ineligible for an HSA.

CNO provides two comprehensive medical options. Both options provide you with the opportunity to open a Health Savings Account (HSA) to set aside money for your current or future qualified medical expenses. You also have the opportunity to earn company-provide HSA contributions by completing various wellness activities (see "Wellness" for further details).
Summary of Benefits and Coverage|
- Medical Option 1 with CCN access | For participants enrolled in Medical Option 1 who have access to Cigna's Open Access Plus network and the Cigna CCN network for various medical specialites.
- Medical Option 1 without CCN access | For participants enrolled in Medical Option 1, who have access to Cigna's Open Access Plus network, but do not have access to the Cigna CCN network for various medical specialties.
- Medical Option 2 with CCN access | For participants enrolled in Medical Option 2, who have access to Cigna's Open Access Plus network and the Cigna CCN network for various medical specialties.
- Medical Option 2 without CCN access | For participants enrolled in Medical Option 2 who have access to Cigna's Open Access Plus network, but do not have access to the Cigna CCN network for various medical specialties.
- Out-of-Area Medical Plan Option | For participants who live in an area where there is no Cigna Open Access Network access available and no access to the Cigna CCN network for various medical specialities.

Health care and dependent care FSAs allow you to set aside pre-tax dollars to help pay for your health and dependent care (i.e. child care) needs. After you incur eligible expenses and submit appropriate receipts with your completed claim form, you'll be reimbursed with tax-free dollars from your account.
Deadlines for Claims Submissions: You have until December 31st of each plan year to incur eligible claims. You must submit those claims for reimbursement by March 31st of the following year. Otherwise unused funds will be forfeited.

All associates are provided with company-paid life insurance in the amount of 1 times their base salary. This is an automatic benefit - you do not need to elect this benefit, nor do you have to submit evidence of insurability.
Associates also have the option of electing supplemental life insurance for themselves, their spouse, and/or their children. (Note: In order to elect supplemental coverage for a spouse or child(ren), the associate must also elect supplemental coverage for themselves.) Details regarding coverage amounts, premiums, and evidence of insurability rules can be found in the Benefits Guide.

The CNOSave Plus 401k plan is provided to help CNO associates save tax-deferred funds for retirement. Prudential is the administrator for our 401k plan, and they offer many investment options, ranging from conservative to agressive, to help you meet your investment goals. The Transportation Savings Plan (TSP) is a tax code provision that allows you to redirect a portion of your wages, on a pre-tax basis, to pay for work-related parking and/or mass transit commuting expenses. To receive these benefits, you must elect the amount you want to contribute up to the statutory dollar amount limits. In turn you will be reimbursed for eligible expenses up to that amount on a tax-free basis. Eligible expenses include: commuter trains, park and ride, buses, parking, and van pooling (at least six passengers).
